義大利和美國有著熱情友好的關係。自1840年以來，美國在義大利及其前身義大利王國設有外交代表處。然而，在 1891 年，意大利政府斷絕了外交關係，並短暫考慮對美國開戰，作為對未決案件的回應11 名意大利人在路易斯安那州新奧爾良被私刑處決，1941 年至 1943 年關係破裂，當時義大利和美國處於戰爭狀態。

第二次世界大戰後，意大利成為一個強大而積極的跨大西洋合作夥伴，它與美國一起尋求在衝突和國內衝突地區促進民主理想和國際合作。為此，意大利政府與美國合作制定國防、安全和維和政策。根據北約成員國的長期雙邊協議，在維琴察和里窩那（軍隊）有重要的美國軍隊駐紮；阿維亞諾（空軍）；和Sigonella、Gaeta和那不勒斯——美國海軍第六艦隊母港。美國約有11,500名軍事人員駐紮在義大利。意大利在羅馬舉辦了北約國防學院。

除了密切的政府、經濟和文化聯繫外，根據皮尤研究中心的全球民意調查，義大利是世界上最親美的國家之一，2002 年有 70% 的意大利人對美國持好感，增加到 78% 根據2012年美國全球領導力報告，51%的意大利人認可美國在奧巴馬政府領導下的領導地位，16%的人不贊成，33%的人不確定。另一方面，許多美國人也對意大利持好感，超過70-80%的美國人認為意大利是最喜歡的國家。
